An observation: you started this thread with " Running Lean P0145 + P0141".
These codes stand for "O
2 Sensor Heater Circuit Malfunction (Bank 1, Sensor 2)." The do not stand for "Running lean". Running lean would be P0171, P0172.

.....as I said, just an observation.

yeah, i posted the wrong numbers, couldn't edit the thread title, i corrected to the lean numbers in the first post. sorry about that.

Chevron. the cheaper one..forget if it was the fuel system or fuel injector cleaner I was using. it was only $4.99.

ok, i knew it was too good to be true, the light came on today. I figured it would after posting that update yesterday...but it was off for quite some time. same conditions, right after i start my car, about 15-30 seconds after I start accelerating after starting the car. I'm thinking the problem is transient as well, as whenever I did real time tests on the system, everything was operating at spec. I guess I'll have to go through the freeze frame data. I'll go through the thread again and try to come up with a plan and summary of everything.

won't have time to go to the mechanic for a week or so since I'm going out of town for a bit.

any interesting theories? I'm thinking its fuel system related, and I'll try to get that fuel pressure test, I would have last weekend but didn't have any CEL code going on and it appeared to be working ok. I'm going to try another bottle of redline, should I up my gas to the 91 octane?

Summary:

Items Fixed:
1. Exhaust Leak- This was found to be a broken bolt in the exhaust manifold causing a leak. Fixed. I've double checked the exhaust and they found no leak at a muffler shop.
2. Rough start, vibration in the AM- This was found to be a bad engine mount. Fixed and the problems are gone now.

Tests to do/look for:
1. Fuel Pressure Check- perhaps a bad fuel pump or fuel filter or stuck/bad injector
2. Reprogram PCM? Have to go to dealer see if I'm missing any PCM updates/recalls.
3. Vacuum leak check- found none with previous smoke test. Code not present at idle when looking at freeze frame data. Will have to look at the following possibilities- Intake Plenuem Gasket, throttle body gaskets. maybe just do another smoke test.
4. Clean EGR
4. Backpressure/Pre-cats check- to do.
5. Clean MAF again, reset PCM by disconnecting battery?

Plan for now:
1. Going to do another bottle of redline fuel system cleaner.
2. Reset PCM by unhooking the battery.
3. Using higher octane chevron gas which seemed to help it out
4. If code is still here in a week or so, I will get the fuel pressure checked and vacuum leak check again to see if any gaskets are leaking.

may pick up a code reader, i don't have my own, it might be helpful.

Redline is safe for continuous usage as per link. Re-setting the computer is good to do especially when changing Octane. 91 Octane is not necessary. I am not the Gas expert. Maybe "Shipo" could give some input on this.

I read you shouldn't reset the computer when using additives because as it trys to "relearn" your driving habits, it will adjust to the fuel with the additives in it. something i saw when looking at the information on this stuff.

Yeah, BUT you are changing Octane!

You may have to do it once you fill up with just 87 again!
